Will My Treatment “Stick”? A Metaphor for Recovery and Challenges Through the Flow of Life

One of the most common questions I hear from patients is, “Will my treatment hold?” or “Will I regress?” It’s a fair question, and while health is always in motion, here’s the good news:

For the vast majority of my patients, once we get you where you need to be, you never need to see me again.

That’s right. Most of my patients experience long-lasting results and continue living their lives without any setbacks. My goal isn’t just to treat you—it’s to equip you with everything you need to stay well on your own.

To help explain this, I like to use a metaphor: life is like a stream, steadily flowing toward a waterfall.

The Flow of Health and Recovery

When you're healthy, the stream moves at a slow, manageable pace. You have the strength to paddle against the current and stay in a good place, maintaining your health with relative ease.

But sometimes, things happen that change the flow. A concussion or another health event is like a sudden heavy rainstorm—it speeds up the current and pushes you further downstream, closer to the waterfall. You may feel like you've lost control, struggling to paddle back up. My role in treatment is to motor you back upstream—back to where you were before the storm, or in many cases, even farther upstream than you were before your injury.

For nearly all my patients, that’s it. Once you’re back upstream, you stay there. As long as life stays stable and another storm doesn’t hit, you won’t need further treatment.

And just in case, I give you exercises—"your paddle"—to help you stay in control of the current.

Why Some People Drift Faster Than Others

For some patients, paddling and maintaining progress is straightforward. For others, the current feels relentless, pulling them downstream despite their best efforts. Why does this happen?

Underlying Conditions Speed Up the Current. If you have an infection, an autoimmune disease, chronic inflammation, or other medical conditions, it’s like other streams flowing into your stream—it makes it harder stay healthy, because the current speeds up. Even after I help you upstream, you might find yourself drifting back down faster than expected. In these cases, we need to identify what’s contributing to the accelerated flow and maybe build a dam to slow the current (address the other underlying health issues).

Neurodegenerative Conditions Change the Landscape. For patients with neurodegenerative conditions like dementia or Parkinson’s, the current is constantly increasing. No matter how strong the motor or how hard you paddle, the water will keep moving. In these cases, we may need to motor you back upstream a couple of times per year, keeping you as far away from the waterfall as possible for as long as possible.

    Your Paddle: Staying in Control of Your Health

    The good news is that you are not powerless in this process. Even if the stream is flowing, you have a paddle. Your paddle represents:

    ✔ The exercises I give you
    ✔ The lifestyle habits you adopt
    ✔ The preventative care we put in place

    If you use your paddle consistently, you’ll be able to stay upstream longer and maintain the benefits of your treatment. If you find yourself drifting, that’s a signal that we need to look deeper—whether it’s addressing an underlying condition, making adjustments to your routine, or returning for another boost upstream.

    The Journey Continues

    Health isn’t a destination—it’s a journey. No matter how young or healthy you are, staying healthy takes workd. Just like a stream never stops flowing, your body is always stressed and adapting to new challenges. My goal is to not only motor you back upstream when you need it but also teach you how to navigate the currents, anticipate storms, and keep yourself as far from the waterfall as possible.

    So if you ever find yourself wondering, “Will my treatment hold?”, remember: as long as you keep paddling and we keep working together, you’ll stay in control of the flow.
