Once we are confident that we are a good fit for each other, we will find a date that works for both your schedule and ours. Dr. Antonucci has limited availability so you will be required to place a $1,710 deposit on a credit card to hold the appointment. The deposit is fully refundable for 72 hours and may be partially refundable thereafter. The terms of the deposit will be presented at the time of payment.

While we pride ourselves on our outcomes and compassionate care, education is also one of our core pillars. In order to enroll you as a patient, we need you to be educated on and aware of the policies of the practice, the type of services we provide, how we handle payments, our expectations, and how we manage your private information. 

Once you schedule your intake date, these will be sent to you and requested that you sign them online. 

If you have any questions about the terms or policies we will happily answer them. 

Once your intake date is confirmed, we will send you a link to our on-boarding center, which will contain instructions on how to complete your intake forms. We recommend completing them within 72 hours of securing your intake date. If your forms are completed within 72 hours and the doctor sees something that leads them to believe he can't help you, you are eligible for a full refund. It will also enable him to order any testing that may help serve you best, before you arrive. We tried our best to limit the number of repeat questions, and apologize in advance if you have to repeat any data input.

These forms will take anywhere between 20-60 minutes to complete depending on your answers. There are multiple forms, but we suggest completing them in one sitting.
